Aimed at share and spread of best practice, the day will focus on four key areas:
- Person centered planning
- Lessons learned from Valuing People
- Web based planning tools
- Mind Mapping person centered reviews
The day will offer you an opportunity to action plan for the next steps in relation to improving outcomes for young people. Delegates will then come together in locality groups for workshop exercises focussing on driving forward the transitions and personalisation agenda across the region.
The day is free to attend, but we are asking that all areas bring a display for our lunchtime exhibit on good practice in transition from each locality.

- Health: Children's commissioners, adult commissioners, service managers, communication managers, transition leads.
- Education: Special school head teachers, SENCOs, commissioning managers, transition leads.
- Local Authority: Children's commissioners, Aiming High commissioners, communication managers, adult services commissioners, service provision managers, transition leads, housing, further education, Connexions managers, benefit agency.
